1.Procedure of Moving In and Out

1-1 Moving In

When moving in to Minokamo City, bring the Moving Out Certificate that the previous municipality issued at the Residents Affairs Division and submit a Moving In Notification.

1-2 Moving Into the Country

Your Residence Card (or similar card), passport, or an official document (Japanese translation is necessary if the documents are not in Japanese.) that indicates relationship to the householder such as original copy of birth certificate are necessary.

A proxy letter will be needed if someone other than the person moving in or someone from a different household does the procedure.

1-3 Moving Out

Bring an identification card and submit a Moving Out Notification at the Residence Affairs Division 14 days before or after moving.

1-4 Moving Out of the Country

A proxy letter will be needed if someone other than the person moving in or someone from a different household does the procedure.

※For questions regarding the period of time out of the country, please consult the Division.

2.Types of Procedures

2-1 Marriage Notification

When getting married, submit a Marriage Notification to the City Hall.

―Marriage between Foreign Nationalities 

Confirm the procedures with the embassy or consulate of your country in Japan as procedures on marriage between foreign nationalities vary by countries. As for marriage procedures in the City Hall, confirm the necessary documents with the City Hall.

2-2 Divorce Notification

Consult the embassy or consulate of your country in Japan as divorce procedures vary by countries.

Divorce Between a Foreign National and a Japanese Divorce by Agreement

Within 10 days of the conciliation being approved.

(5) Confirmation certificates and certified copies of documentation of the mediation process, such as a mediation report, a court judgement or ruling, is necessary for Conciliation Divorce

2-3 Birth Notification

When a child is born in Japan the father or the mother must submit a Birth Notification to the Residents Affairs Division within 14 days after giving birth. (However, the signature of the mother is necessary if the parents are not married)

2-4 Death Notification

A Death Notification must be submitted within 7 days of a death. (Fines may be charged and a Letter of Reason is necessary if late.)

2-5 Personal Seal Registration

This registration is required to receive a Seal Certificate used when borrowing money from a bank and other procedures.

  • A seal written in the alphabet (romaji) that reflects the applicant’s name is permitted.
  • If the seal is written in kanji or katakana etc. that is different from the person’s legal name, two documents (an Employment Certificate, Salary Statement, Employee ID, Insurance Card, Patient Registration Card etc.) that shows the alias※ is necessary.

※An alias is a name different from the real name used in everyday life.

2-6 My Number

(1) Notification Card

A Notification Card is a paper card that shows person’s name, address, birthdate, gender, and “my number” (a 12-digit number). It will be sent to the resident’s address by registered mail.

Bring the Notification Card at the Residents Affairs Division if there are changes in the contents of the card such as change of address etc.

The Notification Card has no expiration date but procedure of re-issuance is necessary if the card has been lost (a fee applies).

(2) My Number Card

A My Number card is an IC chip card that shows a person’s name, address, birthdate, gender, and photo on the front; and indicates a person’s My Number (individual number) and other information on the back. The initial card issuance is free. Please update your My Number Card when there is a change in the visa validity period.

Aside from being used as a document to prove a person’s identity or “my number”, it may also be used for various purposes such as to file income tax returns via the National Tax Electronic Filing and Payment System (eTax) using the IC chip that’s embedded in the card. Please keep it in a safe place.

Please bring the My Number Card to the Resident Affairs Division for any changes in the registered contents such as address change, etc.

Application and Issuance of My Number Card

Please affix your photo to the My Number Card Issuance Application Form, use the enclosed envelope, and send it by mail. 

Minokamo City will send an “Issuance Guide Notification Letter” by post once the My Number Card is ready to be issued. Contact the Resident Affairs Division and inform them of the appointment number that’s highlighted in pink within the letter. On the appointed day, please go to the Resident Affairs Division and bring the Issuance Guide Notification Letter, My Number Notification Card, Residence Card, and other proof of identification.
