13.Medical Welfare Card

This system subsidizes out-of-pocket medical costs from check-ups at medical institutions for infants, children, people with severe disabilities, and single mothers or fathers. (Medical expenses that are not covered by medical insurance will not be subsidized such as medication containers, doctor’s certificates, hospital room charge)

For infants and children Residents of Minokamo City and members of health insurance for children under 15 years old. (Until March 31 of the year when the child turns 15) (1) Health Insurance Card

(1) Medical welfare card
(2) Receipts of medical expenses (With details of name, consultation date, insurance points and seal from medical institution. Ordinary receipts will not be accepted)
(3) Proof of bank account (Passbook or ATM card. Necessary for the initial application only)
For single mothers
  • Single mothers or legal guardians whose combined household income for the previous year (or the year before between January and September) does not reach a set amount, and
  • have a dependent child under 18 years old (until March 31 of the year after they turn 18.) OR
  • Parentless children until March 31 after they turn eighteen
(1) Health Insurance Card
(2) Personal seal (that is not self-inking)
(3) Income tax certificate (Income tax certificate from the previous address if your address was not registered in Minokamo City on January 1st of the current year)
(4) Basic Survivor Pension (Only for receivers of Basic Survivor pension by bereavement)
For single fathers Single fathers or legal guardians whose combined household income for the previous year (or the year before between January and September) does not reach a set amount, and
have a dependent child under 18 years old (until March 31 of the year after they turn 18.)
Persons with severe disabilities
  • People who have a physical disability booklet levels 1 to 3,or specified by the national pension act as level 4.
  • People who have a rehabilitation booklet with A1, A2, B1 level.
  • Persons who have a level 4 of war victim physical disability booklet
  • Persons who have a level 1 or 2 health and welfare care for mental disorder booklet

14.Disability Booklet

A Disability Booklet that indicates disability level and current condition is issued to disabled people and enables them to receive various welfare services.

15.Daily life Consultation(Emotional and Life Consultation Counter)

Consultation is offered to local citizens who are having trouble or have worries. Their concerns will be listened to in detail and professional support will be provided in accordance with the law.

Independence Consultation Support (“Jiritsu soudan shien jigyou”) Consultations and support for life troubles such as societal withdrawal, school non-attendance, developmental disorders, settlement of debts, job hunting support, and others.
Work preparation support (“Shuurou junbi shien”) Providing work experience activities to those who are worried or anxious about participating in society to help them connect with society and gain confidence
Household finances improvement support (“Kakei kaizen shien jigyou”) By supporting the individual to learn, think and systematically manage the balance between income and expenses that will help in acquiring the ability to have a stable foundation of living.
